Please Note: You must fill out a submission form for each video that you submit. Please enclose a $10 processing fee for 'each title'. You do not have to send separate video tapes for each video. You may put more than one video on one videotape, but you must include separate submission forms and processing fees for each video title submitted for consideration. Videos will not be returned so please don’t send masters.
